1. Personalized Care Plans
  • Comprehensive Assessment: Conducting a thorough assessment of the client's medical, emotional, and spiritual needs to create a personalized care plan.

  • Regular Review and Updates: Continuously reviewing and updating the care plan to ensure it meets the evolving needs of the client.

  • Collaboration with health care professionals, palliative care nurses and family.

2. Pain and Symptom Management
  • Pain Relief: Following care plan directives from health care professionals, advise if medication changes are required. 

  • Symptom Control: Reporting any changes in symptoms to clients health care team and family.

3. Emotional and Psychological Support
  • Our caregivers have a compassionate supportive approach. Caregivers are trauma informed. 

  • Advising and giving resources to the family and clients to seek professional psychological/emotional support.

4. Spiritual Care
  • Spiritual Support: Respecting and supporting the client's spiritual beliefs and practices, providing access to spiritual advisors if desired.

  • Legacy Projects: Assisting clients in creating legacy projects such as memory books, letters, or recorded messages for their loved ones.

5. Personal Care Assistance
  • Daily Activities: Assisting with activities of daily living such as bathing, dressing, grooming, mobility and toileting to ensure the client's comfort and dignity.

  • Nutrition and Hydration: Providing assistance with meal preparation and feeding, ensuring the client receives appropriate nutrition and hydration as required.

6. Coordination of Care
  • Collaboration with Healthcare Providers: Coordinating with doctors, nurses, and other healthcare professionals to ensure seamless and comprehensive care.

  • For End of Life Care, scheduling will be flexible and accommodating.

7. Comfort Measures
  • Environment: Creating a peaceful and comfortable environment for the client, including managing lighting, temperature, and noise levels.

  • Personal Preferences: Respecting and honoring the client's personal preferences and routines to enhance their comfort and well-being.

9. Respite Care
  • Temporary Relief: Offering respite care services to give primary caregivers a break while ensuring their loved one continues to receive high-quality care.
